Microneedling Plus Minoxidil: the 2025 Meta-Analysis of 12 Trials

Does adding microneedling to minoxidil really grow more hair, and do the details, like needle depth or device type, actually matter? A 2025 systematic review and meta-analysis, the newest and largest on this question, pooled 12 randomised controlled trials covering 631 people with androgenetic alopecia to find out. The combination clearly beat minoxidil alone on both hair count and hair thickness. And needle depth, treatment duration and device type made no significant difference to the result: what mattered was doing the needling at all.

Why does this study matter?

The idea that microneedling boosts minoxidil is not new. The landmark Dhurat 2013 trial put it on the map, and the Abdi 2023 meta-analysis confirmed the pattern across multiple trials. But individual trials disagree on the details: how deep should the needles go, how long should you keep it up, and does a motorised device beat a simple roller?

Ahmed and colleagues set out to answer exactly those questions. Their review, registered in advance and searching six databases up to September 2024, is the largest pooled analysis of microneedling plus minoxidil published to date, and it ran separate subgroup analyses on depth, duration and device type. What did the researchers do?

The team identified 12 randomised controlled trials with 631 participants aged 18 to 60, all with androgenetic alopecia (male or female pattern hair loss). Every trial compared the same two things: microneedling combined with topical minoxidil versus minoxidil on its own. Eleven of the 12 provided data suitable for the pooled statistical analyses.

The main outcomes were hair count and hair diameter, measured before and after treatment. The researchers also pooled investigator and patient ratings of improvement, collected every reported adverse event, and split the hair count data three ways: by needle depth (1 mm or less vs more), by duration (12 weeks or less vs longer), and by device type (motorised electrodynamic devices vs rollers).

A word on those SMD numbers, because they are easy to misread. SMD stands for standardised mean difference. It is a statistical effect size, not a count of hairs, and by convention an SMD above 0.8 is a large effect. The 1.32 for hair count therefore means the combination produced a large improvement over minoxidil alone, not "1.32 more hairs". The 0.34 for hair diameter is a smaller effect, but it came with zero statistical inconsistency between trials, which makes it one of the most reliable findings in the paper: the combination does not just add hairs, it thickens them. And the improvement was visible to human eyes, not just to counting software: doctors and patients alike were about 5 times more likely to rate the hair as improved with the combination.

Does needle depth or device choice matter?

This is the finding we think deserves the headline. Trials using needles deeper than 1 mm showed an effect size of 1.37; trials at 1 mm or shallower showed 1.25. That difference was nowhere near statistically significant (p = 0.85). Motorised devices edged rollers (1.44 vs 1.17), but again the difference was not significant (p = 0.72). Shorter and longer treatment periods told the same story.

In plain terms: across the published trial record, nobody has shown that going deeper, needling longer or buying a fancier device reliably beats a sensible standard protocol. The variable that separates responders from non-responders is not the gadget; it is whether needling is combined with an active topical and done consistently.

Limitations to keep in mind

The hair count result came with high statistical heterogeneity, meaning the size of the benefit varied a lot between trials even though its direction was consistent. The trials were conducted mainly in Asia, the Middle East and North Africa, with no Caucasian study populations, and results could not be analysed by age. None of this undermines the central finding, but it does mean the true average benefit is estimated imprecisely.
