Quick answer

Microneedling does regrow hair — but the evidence is strongest when it is used alongside a topical treatment, not on its own. In the landmark trial, men who added weekly microneedling to minoxidil gained 91.4 hairs per cm² over 12 weeks; minoxidil alone gained 22.2. The variables that matter most are depth, frequency and persistence — not which device you buy.

  • It works as an add-on. Every large analysis showing a clear benefit tested microneedling combined with a topical. The one trial of microneedling entirely on its own found no gain in hair density.
  • 0.5 mm is the sensible default. The only trial that compared depths directly found the shallower setting did better.
  • Once a week is enough. More often is not better, and one analysis found that reducing session frequency while treating for longer was associated with more hair.
  • Judge it at 12 weeks, not 3. New hair typically becomes noticeable at around week six.

Scalp microneedling is the use of fine needles to make controlled micro-punctures in the scalp, triggering the body's wound-healing response around the hair follicle. It is used for androgenetic alopecia — male and female pattern hair loss — most often alongside a topical treatment such as minoxidil.

Does microneedling actually regrow hair?

Yes — with an important qualification. Microneedling has repeatedly increased hair count in randomised trials of androgenetic alopecia, and the effect is large. But almost all of that evidence comes from studies in which microneedling was added to an existing topical treatment.

The clearest single result is Dhurat et al. 2013, which randomised 100 men with mild to moderate pattern hair loss to twice-daily 5% minoxidil, or the same minoxidil plus weekly microneedling with a 1.5 mm roller.

Twelve men who had previously failed to respond to finasteride or minoxidil did respond once microneedling was added.

Later syntheses agree on the direction. A 2024 meta-analysis of 13 randomised trials and 696 patients found combined microneedling therapy significantly better than any single treatment for both hair density (mean difference 13.36, 95% CI 8.55–18.16) and hair diameter. A separate review of ten trials in 466 patients found that every one of the eight studies it pooled showed a statistically significant increase in total hair count when microneedling was added to minoxidil — though it also found the evidence did not support a significant increase in hair diameter.

Here is the part most pages leave out. When Kakizaki et al. 2023 tested scalp microneedling on its own — 30 men, four monthly sessions, no topical alongside it — they found no benefit in hair density at all, with either device they used. The trial was small and used a much lighter schedule than the protocols that worked, so it is not proof that microneedling alone is useless. But it is the most honest answer available to "does this work without minoxidil?": nobody has yet shown that it does.

The 2022 review by Gupta and colleagues does point the other way — its regression analysis put microneedling monotherapy ahead of topical 5% minoxidil (β = 12.29). Read together, the fair summary is: microneedling is well evidenced as an amplifier of a treatment that already works, and unproven as a treatment by itself.

How does microneedling work on the scalp?

It works by injury. Fine needles make controlled micro-punctures in the scalp, and the body's repair response — not the needles themselves — is what affects the follicle.

The dermal papilla, at the base of each follicle, is where many hair-growth genes are expressed. Wounding the skin above it triggers a cascade of growth factors and Wnt signalling proteins, raises local blood flow, and activates the stem cells associated with the dermal papilla. That is the mechanism the researchers behind the landmark trial set out to exploit, and it is why microneedling is described as a stimulus rather than a drug.

There is physical evidence that the tissue genuinely remodels: when Kakizaki's team took scalp biopsies before and after a course of microneedling, they found neocollagenesis — new collagen formation — and elastolysis in the treated skin.

It is worth being straight about the limits of this picture. As the 2022 review puts it, the exact mechanism of microneedling "is yet to be determined". We know that wounding the scalp changes what the follicle does. Precisely which part of that cascade produces the hair is still an open question.

One thing microneedling is not: an injection system. The U.S. Food and Drug Administration states that microneedling devices are not approved for delivering cosmetics, topical medications, vitamin solutions or drugs into the skin. Apply your serum to a clean scalp as part of your routine. Do not think of the device as a way of pushing product deeper.

Which device should you use — roller, stamp or pen?

For the scalp, a stamp or a pen is easier to use well than a roller — but the choice matters far less than the depth you set.

The evidence base sits mostly with rollers, for a reason that is about history rather than performance: the roller was the cheap, widely available at-home device when the hair-loss trials were designed in the 2010s, so it is what researchers put in patients' hands.

When someone finally ran the comparison, the devices came out level. Kakizaki 2023 randomised its 30 participants to a roller or to a vertical-entry tattoo cartridge — mechanically the same straight-in, straight-out action as a stamp — and neither device outperformed the other.

Where the pen earns its place is control. It is the only one of the three where needle depth is a setting rather than a fixed property of the head — and depth is the single variable with head-to-head trial evidence behind it. That is an argument about precision, not about the device being inherently more effective; no trial has compared these device types and found a winner.

What needle depth should you use for hair growth?

Start at 0.5 mm. It is the depth with the best evidence behind it, and it is comfortable enough that people actually keep going.

Faghihi et al. 2021 is the only trial to compare depths head-to-head. Sixty patients were split three ways — minoxidil alone, minoxidil plus microneedling at 1.2 mm, and minoxidil plus microneedling at 0.6 mm, every two weeks for 12 weeks. The 0.6 mm group was significantly better than the control group on both hair count (P = 0.017) and hair thickness (P = 0.007), and the authors concluded that this shallower depth "tended to be more beneficial" than 1.2 mm.

That is a genuinely surprising result, and it is worth stating carefully: 0.6 mm did not beat 1.2 mm by a statistically established margin. It trended better. What the trial rules out is the common assumption that deeper must mean more.

Note that 1.5 mm is not a rogue setting: it is exactly what the landmark trial used, weekly, with no significant adverse effects reported. Depth and frequency are a package.

What is not supported is going deeper than the trials went. 2 mm and above has not been tested for hair growth in any of the studies above, it raises the risk of bleeding, pigment change and scarring, and it has no demonstrated payoff. Scarring is the one side effect that permanently destroys follicles, which makes it the wrong risk to take casually.

How often should you microneedle your scalp?

Once a week suits most people at 0.5 mm. The protocols that produced results in trials ran weekly (Dhurat, at 1.5 mm) or every two weeks (Faghihi, at 0.6 mm). Neither went more often than that.

More is not better, and there is data behind that. The 2022 review found that increasing the overall duration of treatment while reducing the frequency of sessions was associated with a greater increase in total hair count. The scalp needs to finish healing between sessions; if you needle again before it has, you are maintaining an injury rather than completing a repair.

The simplest test of whether you are overdoing it: if redness has not settled within about 24 hours, you are going too deep, too often, or both. Persistent inflammation is not evidence that it is working.

Going the other way is a real failure mode too. Kakizaki's monthly schedule is the one that produced nothing.

What should you use alongside microneedling?

A topical minoxidil routine, if it suits you — that is the pairing the evidence is built on. Every large positive result described above tested microneedling plus a topical, and the 2022 review estimated the added benefit of putting 5% minoxidil on top of microneedling at β = 7.63.

A practical note that is easy to get wrong: do not apply minoxidil immediately after needling. Freshly needled skin is far more permeable and far more easily irritated, and minoxidil solutions commonly contain propylene glycol, which stings badly on broken skin. Common clinical practice is to leave 24 hours between a microneedling session and the next minoxidil application, then resume the normal routine.

Copper peptides are the other pairing people ask about, and it is the one we make, so we will be careful here. What does not yet exist is a trial isolating copper peptides with microneedling. The closest is a 2025 study of five monthly sessions that reported enhanced regrowth — but its protocol combined minoxidil, dutasteride and copper peptides in one preparation, so the copper peptide's independent contribution cannot be separated out. That is an honest limitation rather than a reason to avoid the combination; it means the claim "copper peptides plus microneedling is proven" would be going beyond the data.

Whatever you pair it with, keep the FDA's position in mind: the device is a stimulus, not a delivery mechanism.

How long does microneedling take to show results?

Judge microneedling at 12 weeks. That is where the trials measured, and it is where the effects showed up.

Two things follow from that table. First, a three- or four-week trial of microneedling tells you nothing; the shedding-and-regrowth cycle is simply longer than that. Second, the gains reported in these studies came from finishing a protocol, not from an intense start. Consistency is the active ingredient.

What are the risks, and who should not do this?

The U.S. Food and Drug Administration lists the common risks of microneedling devices as bleeding, bruising, redness, tightness, itching and peeling, usually settling within days. Less common risks include infection, pigment changes and cold sore flare-ups.

The trial evidence is reassuring on safety at sensible depths. Dhurat reported no significant adverse effects in either group. The 2023 review of 466 patients reported no scarring and no serious adverse events. The 2024 meta-analysis of 696 patients found no statistical difference in adverse events between combination therapy and single treatments.

Do not microneedle your scalp if you:
  • have a bleeding or clotting disorder, or take blood-thinning medication
  • have an active scalp infection, open wounds, or inflamed psoriasis or eczema on the scalp
  • have a history of keloid scarring
  • are being treated for a skin cancer on the scalp

The mistakes that cause almost all the harm

  1. Not disinfecting the device. Infection risk comes overwhelmingly from a dirty device. Clean it before and after every session, and never share it.
  2. Using blunt needles. Dull needles tear rather than pierce. Replace heads on schedule.
  3. Going deeper because progress feels slow. The one trial that compared depths found the shallower setting did better.
  4. Needling inflamed skin. If the scalp has not settled from the last session, wait.
  5. Applying an irritant straight afterwards. Give it 24 hours.

Frequently asked questions

Does microneedling really work for hair growth?

Yes, when it is combined with a topical treatment. In the landmark 2013 trial, adding weekly microneedling to minoxidil produced 91.4 extra hairs per cm² over 12 weeks against 22.2 for minoxidil alone, and meta-analyses of 466 and 696 patients agree on the direction. What has not been demonstrated is that microneedling works on its own — the one trial that tested it alone found no gain in hair density.

How often should I microneedle for hair regrowth?

Once a week at 0.5 mm suits most people. The successful protocols ran weekly at 1.5 mm or every two weeks at 0.6 mm — none went more often. One review found that treating for longer while needling less often was associated with more hair, so there is no advantage in going faster.

Is 2 mm too deep for microneedling at home?

For the scalp at home, yes. The trials above went to 1.5 mm at most, and the only study that compared depths found the shallower 0.6 mm setting did better than 1.2 mm. Going beyond 1.5 mm raises the risk of bleeding, pigment change and scarring with no demonstrated benefit, and scarring permanently destroys follicles.

What's better, microneedling or minoxidil?

The question is slightly the wrong shape — the evidence is strongest for the two together. A 2022 regression analysis did place microneedling monotherapy ahead of 5% minoxidil, but the same analysis found that adding minoxidil on top of microneedling improved results again, and the largest trials all tested the combination.

Is it okay to microneedle your scalp every day?

No. Even the gentlest 0.25 mm routine tops out at two to three times a week, and at 0.5 mm and above the scalp needs a full week to heal. Daily needling keeps the skin in a state of injury rather than letting it complete the repair response that produces the benefit.

Can you over-microneedle your scalp?

Yes, and it is the most common mistake. Too deep or too frequent leaves the scalp persistently inflamed and, in the worst case, scarred — which destroys follicles for good. If redness has not settled within 24 hours, reduce the depth or the frequency.

Who should not use a derma roller or microneedling device?

Anyone with a bleeding or clotting disorder or taking blood thinners, anyone with an active scalp infection, open wounds or inflamed psoriasis or eczema on the scalp, anyone with a history of keloid scarring, and anyone being treated for a scalp skin cancer. If you are unsure, ask your doctor first.

How long does microneedling take to show results for hair?

Expect nothing for the first month. In the Dhurat trial, new hair growth became noticeable at around six weeks in the microneedling group, against ten weeks for minoxidil alone, with the main gains measured at the 12-week endpoint. Judge it at three months, and expect longer courses to do better.
